A short sentence can be a beginning
“Something has changed with my erections, and I would like to talk about it.” You can bring that sentence to a medical appointment without arriving with an explanation. If speaking feels difficult, a note can hold the first few details: when the change began, what you notice and what you want help understanding. There is no need to present a polished account.
